The Ohio Bankers Benefits Trust has been offering quality health benefits exclusively to its members since 1952. Collectively, the OBBT provides benefit options and plans for small to medium-sized banks to assist in competing for and retaining the best employees..

The OBBT strives to maintain a high-quality, cafeteria-style program while keeping premium increases to a minimum through the concept of risk-sharing. Each bank has the opportunity to choose among our many options for medical, dental, vision, life, and other voluntary benefits, and then offer its employees the option of selecting a plan that meets their needs. Each bank will have access to our online eligibility system to simplify the enrollment process by eliminating the need to remit paper forms. To support each bank’s human resources capabilities, the OBBT staff includes a full-time plan administrator who is always available to answer any questions that may arise.

Choice
In response to the demand for consumer-driven health plans, in which members are more involved in the financial aspect of their health care, the OBBT offers HDHP Plans that include both HRA and HSA options to its member banks. These options include a PPO network to help the members maximize their health care dollars by accessing preferred pricing. In addition, the OBBT provides additional support through the transparency tools found on Anthem’s web site. Anthem’s Engage mobile app allows a simple and convenient way to view your benefits, find a doctor, check your claims and download your ID card. While most medical plans include prescription drug benefits, OBBT's plan includes an enhanced preventative drugs list for Diabetes and Cholesterol medicines as well as Asthma and High Blood Pressure at no cost to the insured. For additional savings and the convenience of receiving medications at home, maintenance drugs can be filled through the mail order pharmacy.
Manage Costs
One of the goals set by the OBBT for its medical plans is to provide member banks with the tools to enable them to effectively manage the rising costs of health care. As part of the wellness program, we encourage each bank to schedule a free on-site biometric screening so that it is convenient for the employees to take an active role in their own health. Alyfe Wellbeing Strategies conducts the testing and reports the results immediately to the participant after the screening. The results are also provided to Anthem so that the participant’s Personal Health Record may be populated. In addition, online Health Risk Assessments are available for members to complete in order to obtain valuable insights into the risk factors that they themselves have the ability to modify and control in order to improve their overall health. The OBBT is committed to making an impact on the health of the employees of its member banks. Therefore, every employee and spouse enrolled in one of the OBBT’s medical plans may receive a biometric screening at no cost. Active enrollees in any of the OBBT medical plans can view claims status and paid claim details as well as EOBs (Explanation of Benefits) online through Anthem's secure, web-based personalized member portal. Members may also estimate cost of care, access various discount programs, participate in online wellness and disease management programs, maintain a personal health record, and much more.

The OBBT offers two dental plans through leading provider Delta Dental that can enhance your benefit package, either when combined with the medical plan or on a stand-alone basis. Both plans include a passive PPO network. The employees are not required to use network dentists in order to receive higher benefit levels. However, if a PPO provider is selected, the network guarantees the member will not be balance-billed for charges above the dentist’s contracted amount, which is often significantly less than the standard usual and customary fees charged by non-PPO providers. Active enrollees in the OBBT dental plans can access benefit information, view claims status and paid claim details as well as EOBs (Explanation of Benefits) online through Delta Dental, a secure, web-based personalized member portal. Members may also contact customer service 24 hours a day, seven days a week, by using the email option, and expect to receive a response from a representative within 24-48 hours.
Dental 1
Dental 1 is a comprehensive plan that offers coverage for preventive and diagnostic care, restorative services, major services, and orthodontia.
Dental 2
Dental 2 is a lower cost option that provides coverage for preventive and diagnostic care and restorative services only.
The OBBT sponsors a high-quality vision plan through Vision Service Plan. VSP has one of the nation's largest networks, and it includes both independent eye care providers as well as retail chain affiliates. Benefits include a $10 copay for an annual WellVision eye exam; a $20 copay for Primary Eyecare visits; a $25 copay for single, bifocal, or trifocal lenses; allowances for frames and contact lenses; and discounts on LASIK surgery. The plan provides these benefits every 12 months.

Active enrollees in the OBBT vision plan can access benefit information, view claims status and paid claim details, locate a participating provider, and learn about laser vision correction discounts online at www.vsp.com, through the secure, personalized member portal. Scheduling an appointment with a VSP provider is easy. No ID cards are necessary, and there are no claim forms to file.
A variety of life insurance policies available through the OBBT allows member banks to provide employees with basic life insurance as well as offer them the opportunity to further protect their families by purchasing additional coverage.

Basic coverage is available in flat dollar amounts of $10,000 or $15,000, and percentages of salary of 100%, 150%, and 200%. Optional coverage can be offered in addition to basic life coverage at 100% of salary. The guaranteed issue amount is $300,000, and the maximum amount of coverage available for both the basic and optional amounts combined is $450,000. All basic life policies include Accidental Death & Dismemberment (AD&D) and a Waiver of Premium clause. Dependent life insurance in the amounts of $5,000 for spouse and $2,500 for each child is also available as optional coverage.
Section 125 of the Internal Revenue Service code allows employees to pay for their portion of medical plan premiums and specific eligible expenses with pre-tax dollars. This salary reduction actually increases take-home pay because the employee is saving Federal and FICA taxes. There are two types of Flexible Spending Accounts, Health Care Reimbursement and Dependent Care Reimbursement.
Health Care Reimbursement
These accounts are funded each year with amounts pledged by the employee and deposited on a pre-tax basis. As medical expenses are incurred, the funds are withdrawn from the account to reimburse the employee. Funds not used by the end of the calendar year may roll over up to $500 of unused amounts. These amounts can be used during the following Plan year for expenses incurred in that Plan year.
Dependent Care Reimbursement
These accounts are also funded each year by amounts pledged by the employee and deposited on a pre-tax basis. The annual limit is $5,000. Funds can only be used for reimbursement of expenses for dependent care that allow the employee to work. Funds not used by the end of the calendar year are forfeited. The OBBT plan allows member banks to determine the plan limit. Currently, the only means by which employees are able to be reimbursed is through paper claim submission.

As medical expenses are incurred, the funds are withdrawn from the account to reimburse the employee. Funds not used by the end of the calendar year may roll over up to $500 of unused amounts. The OBBT plan allows member banks to determine the plan limit (up to the max limit of $2,700) and the means by which employees are reimbursed, either through claim streamlining, or automatic reimbursement, or via debit card. Dependent Care Reimbursement accounts are also funded each year by amounts pledged by the employee and deposited on a pre-tax basis. The annual limit is $5,000. Funds can only be used for reimbursement of expenses for dependent care that allow the employee to work. Funds not used by the end of the calendar year are forfeited. The OBBT plan allows member banks to determine the plan limit. Currently, the only means by which employees are able to be reimbursed is through paper claim submission. Participants in the OBBT Flexible Spending Accounts can manage their accounts online. They can calculate savings, view account balances, review recent activity, and explore eligible expenses through Anthem's secure, personalized member portal.
The OBBT has contracted with an outside vendor to provide valuable income protection. Short term and long term disability income plans are available at various benefit levels to assist member banks in meeting their employees' financial needs.

The OBBT would be pleased to consider custom alternatives in addition to the above plan design options. Member banks enrolled in either the STD or LTD plans can confidentially update employee records/status, initiate STD claims, and view premium statements online through the Sun Life web site by registering for a secure access account.
The OBBT is proud to offer its member banks voluntary plans at no additional cost to them. These plans provide employees with the opportunity to purchase specialized benefit plan coverage through the convenience of payroll deduction.
